You can access the issues system in a number of places:
The Diary - shows the active issues that you own
Desktop Issues - shows all active issues
Project Issues - clicking on the project node shows active issues for the project
This shows a typical issues grid:

Right clicking in the issues grid allows you to add, edit, or delete an issue. If the complete option is present it allows you to set the issue status to complete.

This is the Issue Edit Form.

Details - The issue edit allows you to give the issue a description, assign an owner, set the priority, identify the area that the issue applies to, and what sort action the issue relates to.
Properties - Allows you to add or edit extra properties about the issue.
FileStore - The FileStore tab allows you to attach documents to the issue.
Audit Trail - Records changes to the owner of the issue so you can see who has worked on it an notes they've made.
Recording Work
You can record work on tasks by dragging and dropping the task onto your diary. You can set the work to billable if required, otherwise it will just record the cost of the work against the project using the cost rate of the project partner.
